If you are on regular medication the doctor may agree for you to obtain a repeat prescription without needing to see the doctor in surgery. In this case please can you request your prescription in one of the following ways:

You can request your prescriptions online through Patient Access:
If you have not used the service before and have received a letter from us with your PIN number then you can register for an account.
In order to use this service you will need to sign up in advance by completing a registration form. The form is available from either reception at either surgery.
(please note that online repeat prescription request accounts will only be created for patient aged 16 and over*.)
Please return completed forms to reception or by post to the practice at:
*Exceptions can be made for issuing account information to under 16 year olds, for example in the cases of childhood chronic illness.
To request an account on behalf of a patient aged under 16 years old please write to the patients GP. If endorsed by the GP account registration information will be sent to the patient.
