AppointmentsAppointments All appointments to see a nurse or doctor are made by telephoning the surgery on the following numbers: Axbridge Surgery (01934) 732464 Wedmore Surgery (01934) 712774 During normal opening hours, your call will be answered by a receptionist, who will make an appointment for you or arrange a telephone triage call from a GP. When the surgery is closed you will hear a recorded message explaining how you can contact the doctor in an emergency.

[back to top] Change Of Details If you change your name, address or telephone number, please inform the receptionist at the earliest opportunity. We do have a boundary for our practice; please ask at reception for details. Do let us know if you move outside the practice area as it can be difficult for the medical team to visit patients too far afield and in that event you might be asked to join a new practice. If you have any concerns about registering with a new doctor, please discuss them with one of the doctors here or the practice manager. [back to top] Patient Confidentiality In order for you to receive proper care and treatment, we ask you for information which is kept securely within the practice in handwritten medical records and on our computer system. You may be receiving care from people other than the medical team at the practice, both inside and outside the NHS. So that we can all work together for your benefit we may need to share some information about you. We only ever use or pass on information about you if people have a genuine need for it in your and everyone’s interests. Whenever we can we shall remove details which identify you. Everyone working for the practice and the NHS, and anyone who receives information about you from us, has a legal duty to keep that information confidential. If at any time you would like to know more about how we use your information, please ask to speak to the practice manager. [back to top] Access To Medical Records You do have the right, if you so wish, to see and/or have copies of your medical records, both in the written notes and those held on computer. There are certain rules and regulations that the practice has to adhere to when providing this information to you and we may charge you a fee for doing so. Again, if you wish to see any information that we hold on you please ask to discuss this with the practice manager. [back to top] Zero Tolerance By far the vast majority of our patients are extremely considerate and well behaved in their dealings with members of the practice. However, we would like to point out that in line with our NHS colleagues, we operate a ‘zero tolerance’ policy, which means that we do not tolerate aggressive, abusive, violent or threatening behaviour towards any member of staff. Any patient who behaves in this way will be reported to the police and will no longer be able to use the services at this practice. [back to top] Comments We aim to provide the highest possible level of service to all of our patients and we welcome your views, suggestions or complaints to enable us to do so. If you have any comments you would like to make about any aspect of our service please ask to see the practice manager or, alternatively, write to the practice manager or one of the doctors at either surgery. [back to top] Freedom Of Information – Publication Scheme The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available. This scheme is available from reception. [back to top]

Patient InformationNHS 111 is a new service that’s being introduced to make it easier for you to access local NHS healthcare services. You can call 111 when you need medical help fast but it’s not a 999 emergency. NHS 111 is a fast and easy way to get the right help, whatever the time. The service is due for launch in Somerset in March 2013. Click here to see more details regarding this service from the NHS Choices website. From March 2013 NHS 111 will be the number for patients to call for urgent medical care or advice or if they become ill, particularly after their GP surgery has closed. The service is part of the wider improvements now being made to the urgent care system. The aim is to deliver a 24/7 urgent care service that ensures people receive the right care, from the right person, in the right place, at the right time. ResearchResearch Research is a key principle of the NHS to improve the current and future health and care of the population. Research involving patients and members of the public leads to results that are more reliable and therefore more likely to be put into practice. We want patients and the public to be involved in all stages of research. This covers: Setting research priorities Helping to decide what the research needs to achieve Choosing the methods used to carry out research Recruiting people into research studies Understanding what the research findings mean for patients and how it can be utilised in Primary Care The team consists of: Staff Details Dr Ewart Jackson Voyzey Lead Research Investigator. Ceri Walsh Research Nurse. Ceri qualified in 1990. She has come from the Health Protection Agency, employed as a Nurse Practitioner; prior to this she spent 7 years in Swansea setting up the 2nd Regional Cardiac Centre in Wales. Although relatively new to research Ceri has studied for her BSc in Nursing Studies and MSc in Healthcare Management. Rebecca Bale Research Administrator. Rebecca has a vast amount of knowledge in clinical research and has previously worked at clinical trial units within Universities where she has run studies from within the unit. At Birmingham Oxford & Cambridge she has worked on health areas such as breast cancer, low back pain and diabetes. Patients currently registered with the Axbridge & Wedmore Medical Practice may be invited to join studies if they meet the strict eligibility criteria. Patients will be fully informed about any study before they consent to take part in any research. Patient care will never be compromised whether they decide to take part or not. We will keep everybody fully informed and as always put our patients first.
New PatientsNew Patients We encourage all new patients joining the surgery to have a medical examination. Please make an appointment with the receptionist to see the doctor or nurse for your medical. We also request that you fill in a “New Patient Registration Form” and bring it to the surgery when you register. The forms are available at reception.

Test results are available from reception after 2.00pm each day or via telephone consultation with a doctor or nurse.
